Home and Community Support Services
The Waiver is designed to provide as much independence at home and in the community as possible. Home and Community Support Services (HCSS) are the physical and prompting support services that enable the participants to remain healthy and safe, as well as to complete their daily tasks and activities. These services provide cueing and supervision of safety (at home and in the community), and the cueing to complete activities of daily living.
Some of the Services Available
- 1Turning the stove off after use
- 2Prompting taking medications
- 3Paying bills, or making an appointment
- 4Taking care of personal hygiene
- 6Transportation into the community such as banking, shopping, or visiting friends/family
- 7Cooking/Meal preparation
- 10Prompting organizational skills
- 11Prompting good communication skills
- 12Toileting assistance
HCSS is in place to promote as much independence in these activities as possible. For Waiver participants that are able to complete tasks on their own, HCSS is simply there to provide a prompt to complete the tasks when necessary (and provide as much assistance as needed in their completion). For participants that are unable to complete tasks, HCSS will complete tasks in their entirety (with the exception of administering medication).
Our HCSS staff follow a plan of care developed by our Nurse (RN) in conjunction with the Waiver participant’s doctor. The Doctor is also required to be notified and sign off on any changes to the plan of care resulting from changes in needs, the prescriptions, or even the reduction of family supports. HCSS staff write daily notes that are reviewed our nursing staff for any indications of changes in health status. Any concerns will be followed up on by the RN, as well as the Waiver participant’s doctor if necessary.
Home and Community Support Services are scheduled around the participant’s needs, and scheduling can be provided in blocks of hours in the AM, PM, both, or up to 24 hours per day.
